The Role 

This is an exciting opportunity for you to work within our Council and NHS “Good “ rated -  Home Support  and  Reablement Team.  As part of an integrated team working  with other professionals to provide all aspects of care, which include moving and handling , personal care and ensuring the service users dignity and wishes are at the centre of the care provided.

 

This service provides wrap around care and also supports service users being discharged form hospital in a timely manner.   


About You

We seek employees with patience and understanding of how to care for adults,  to enable them to remain in their own homes and live as independently as possible.

 

Successful candidates must ensure that the service is delivered to the highest professional standards in order to support vulnerable adults within their own home, ensuring rehabilitative needs are supported to enable independent living.  Your aim is to help people regain the ability to perform their usual activities, for example cooking meals, washing and dressing, getting about and managing their own medication, all of which promotes independence and positive outcomes.

 

You will need to be able to travel across the borough with access to you own car, to deliver the service and have excellent time keeping, be able to work on your own initiative and have the ability to communicate clearly with service users and other professionals.   

 

The service operates over 24 hour per day , 365 days,  therefore the post holder will be required to work flexibly up to 5 days from 7 days, including evenings, weekends and Bank Holidays on a rota basis. 

About Us –

We are offering the opportunity to become part of a short intensive service that may at times provide support in conjunction with NHS therapy services. If appropriate our support staff will be given therapy led plans to follow.

 

Although regaining physical ability is the primary focus, addressing psychological needs in building confidence as well as social needs and related activities is also vitally important.

 

There are some important differences between Reablement and conventional home care services. Initially Reablement support workers may visit more frequently throughout the day and stay for longer periods. Instead of doing things ‘for’ the person, such as assisting in getting up and dressed, Reablement support workers will work ‘with’ the person to do things for themselves. This means that the support workers are more likely to give encouragement, or show the person a new or better way of doing something in order to encourage independence. This type of support takes longer than conventional home care, which is reflected in the length of time allocated for our Reablement service. By the end of the period of Reablement, the aim is that the length of the visits should have been reduced or no longer be required.
