Social Worker - Integrated Discharge Team

Salary: £29, 323 - £32, 486
Hours: 37 per week

The Integrated Discharge Team is based within Royal Bolton Hospital. The main function of the role is to work as a member of the multidisciplinary team carrying out assessments of need for people over the age of 18yrs, under provision of the Care Act.  Facilitating safe and timely discharges to support patient flow through the hospital and ensuring that the customer is central to the process is essential to the role.


Putting the customer and their family at the heart of what we do with a `Think Home First` philosophy is an essential part of our service. As a multidisciplinary team we work closely with ward staff and other disciplines ensuring that the service we provide is appropriate and safe for discharge.

We have close links with our colleagues in Intermediate Tier and the Community teams and share information as appropriate, to ensure that our customer journey and patient flow is safe and timely.

You will preferably have at least 2 years post qualified experience in Multidisciplinary working – ideally within a hospital setting. You will be familiar with working in a fast-paced environment with the skills to prioritise own work load and be passionate about achieving positive outcomes for individuals, taking into account all services available, including Universal Services to promote independence.

We are committed to safeguarding and promoting the welfare of children and young people and expect all staff and volunteers to share this commitment.  A DBS check is therefore required for this post.
