Adults Assessment and Care management

Working as part of a team this role involves the provision of social work and case management support to Adults aged 18 and above  in Bolton, with a range of needs resulting from ill health, social isolation, poor mental health or safeguarding concerns. 

The key duties of the role include assessment, developing and arranging appropriate services/support for service users, their families and carers, and providing professional social work support. 

The role also involves Investigating safeguarding concerns and the development of protection plans,  working in collaboration with partner agencies such as mental health, district nursing and other health teams, housing and the voluntary sector.

About You:

The Ideal candidate will have previous experience in a social work setting and have experience in completing Care Act Assessments for  adults with a range of needs. 

You will have knowledge and experience in developing support plans and facilitating care arrangements. 

You will have an understanding of the needs of Carer’s and their right to a Carer’s assessment and support.  

The ideal candidate will also have an understanding of working in an integrated way to deliver seamless support to service users and will also have knowledge and experience in Safeguarding.

You will strive for continuous improvement and take responsibility for your own professional development

West Adults District Social Work Team is a busy social work service providing social work services to adults in Bolton.  The service offers the provision of social work and case management support to Adults aged 18 and above in Bolton, with a range of needs resulting from ill health, social isolation, poor mental health or safeguarding concerns. The service operates Monday to Friday.
