The Role:

Grade H/I  Social Worker 

An opportunity has arisen for 1 x permanent Social Worker and 1 x fixed term Social Worker (12 months) within the Intermediate Tier Services. This service covers Admission Avoidance, Intermediate Care bed-based units and Home Support Reablement.

This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote wellbeing and independence. 

The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working. 

About You

We are looking to recruit two enthusiastic and motivated individuals who have a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence. 
The post holder will be working within a team that offer service delivery over 365 days of the year however they will not be required to cover weekends and bank holidays. 

About Us 

The Social Workers in IMC will work within one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people in to universal and voluntary services. 
They will also assist colleges within other areas of the services as necessary and support with complex cases, mental capacity and best interest assessments.
