Service Co-ordinator - Reablement

Contract:  12 month Fixed Term
Grade: 6 plus 7% for out of hours working
Salary: £22, 658.00 - £25, 951.00

An opportunity has arisen for a number of co-ordinator posts within the busy Home Support Reablement Team.

The Reablement services are for people aged 18 and over who are resident in Bolton and have disabilities and/or are frail or recovering from a short term illness or injury. The service is aimed at individuals who are medically stable and safe enough to be supported in their own home and have the potential to improve with a Reablement approach.


Reablement is intended to be a short intensive service, and may be provided in conjunction with NHS therapy services. If appropriate, our support staff will be given therapy led plans to follow. The aim of the service is to help people regain the ability to perform their usual activities, for example cooking meals, washing and dressing, getting about and managing their own medication, all of which promotes independence and positive outcomes.

You will be able to supervise a dispersed team of Home Support Workers within a designated area of responsibility, ensuring  compliance with and improvement upon National Standards, in order to deliver high quality service responsive to individual need. 

                  

You will work with internal/external partners to achieve the best possible outcomes for people. 

The Co-ordinator role involves assisting with the screening and allocation packages of support on behalf of The Home Based Reablement Service and works with computer based information systems that are involved in rostering and planning work. They allocate staffing resources to cover all existing and new requests for support and respond to customer demand from both internal and external partner agencies. 

You must ensure that the service is delivered to the highest achievable professional standards in order to support vulnerable adults with both rehabilitative and / or mental health needs, to retain / regain community living.

 

You will need to be able to travel across the borough to deliver the service and have excellent time keeping.

 

The service operates over 365 days per year between the hours 7.15am – 10.00pm therefore the post holder will be required to work flexibly on a 5 from 7 basis – including evenings, weekends and Bank Holidays.
