Hours- 37 hours per week
Contract- 12 months Fixed Term Contract
Grade-Grade H scp 27 £30,507 to 31 £33,799

An opportunity has arisen to work within Bolton Council’s integrated Community Learning Disability Team which consists of health and social care colleagues. 

Some duties of the role will include:
 
  • Care Act assessments for clients and carers, including the assessment and management of risks, and asset based support planning.
  • Best Interests decisions adhering to Mental Capacity Act.
  • Court of Protection applications, including the principles of Deprivation of Liberty Safeguards. 
  • Resettlement from out of area placements back into their local area.
  • Transition work for young adults with a learning disability transferring between Children and Adult Services, including education, health and care planning.
  • Supporting individuals to move on from their family homes.
  • Safeguarding investigations.
  • Continuing health care assessments and multi-agency meetings.
  • Inter-disciplinary working.

We are looking to recruit a qualified, enthusiastic and motivated individual who has a passion for working with adults with Learning Disabilities, understanding their needs, aspirations and supporting them to enhance their quality of life. You will have the ability to communicate, develop, maintain and improve relationships with service users and carers is essential in these roles as well as the attributes required to participate in inter-disciplinary team working.

The team supports adults and young people in transition with Learning Disabilities.  You will need to have experience in carrying out assessments of need under the Care Act 2014 and devised and implemented asset based support plans, including supporting customers to access universal and voluntary services. You will carry out carer assessments and support plans under the Care Act 2014 and adhere to the principles of the Mental Capacity Act 2005.
