The Role

An exciting opportunity has arisen in Bolton to join the Transition Team working with young people age 14-25 with special education needs and disabilities. The successful candidate will work with both Children and Adult services to ensure a smooth transition for young people in preparing them for adulthood.

 

The social worker will be responsible for undertaking holistic assessments and developing transition plans that will inform the appropriate future pathway and support for the young person.

About You

We are looking to recruit a qualified social worker, who is enthusiastic and motivated with an interest and passion for working with young people with disabilities, understanding their needs, current and future aspirations, and to support them in preparing for adulthood.

 

You will have the ability to communicate, develop, maintain and improve relationships working in collaboration with young people, their families and key partners.

 

You will work with commissioners alongside the Deputy/Team manager to develop the market and ensure there is sufficient, effective and good quality provision in the local area.
